I don't know of any reasonable PDF converters, but the PDF Converter from
ScanSoft at $50 works extremely well. (01)

It is an add-in to Word and allows opening a PDF file in Word, which can
then be saved as a Word file or an HTML file. Word's native HTML is
abominable, but there is a free add-in for Word 2000 to generate clean HTML,
